Understanding the program

What is Family-Managed Home Care?

Family-Managed Home Care (FMHC) is an Ontario Health atHome program that enables eligible patients or their substitute decision-makers to arrange approved home-care services within an authorized care plan and funding agreement.

Families take responsibility for selecting care providers, coordinating services and meeting the program’s funding and reporting requirements.

York Home Care can support families by providing nursing and personal-support services, scheduling, clinical oversight, timesheets and itemized invoices.

Important distinction

York Home Care does not determine eligibility, approve funding, administer government funds or guarantee reimbursement.

Who may be considered

FMHC is designed for specific patient groups

A patient must first qualify for traditional home care and have an Ontario Health atHome care plan.

An assessment is required

Being in one of these groups does not automatically mean approval. Ontario Health atHome assesses care needs and program eligibility and develops a service plan. A substitute decision-maker’s ability to manage the program’s responsibilities is also assessed.

Contact Ontario Health atHome

​Speak with your existing care coordinator or call 1-833-515-1234 to ask about an assessment and Family-Managed Home Care.

Complete the assessment
Ontario Health atHome assesses care needs, program eligibility and the patient’s or substitute decision-maker’s ability to manage the required responsibilities.

Confirm the approved care plan
If approved, clarify the authorized services, funding, provider requirements, allowable expenses and reporting obligations.

Discuss staffing with York Home Care
We review your approved plan with your consent and prepare a nursing or PSW proposal. Before services begin, confirm provider eligibility, rates and payment arrangements with your care coordinator.

Please do not upload care plans or medical records through our general website form. Contact us to arrange a secure way to share information.

With consent, our RN reviews the approved services, schedule and provider requirements, then prepares a staffing proposal.

Services are subject to staffing availability and confirmation of allowable expenses. Keep existing care arrangements in place until a transition plan and start date are confirmed.

Backup and contingency coverage

A Plan for When Care Arrangements Change

Staff illness, unexpected absences and changing care needs can interrupt routines. York Home Care works with your family to establish a practical contingency plan before care begins.

Backup team

Alternate caregivers are identified and receive patient-specific orientation where staffing permits.

Call-in process

Families receive timely notification of staffing changes, contact details and clear escalation steps.

Essential-care priorities

The plan identifies care tasks that cannot safely be delayed and agreed alternatives if replacement staff are unavailable.

Clinical escalation

Staff follow patient-specific instructions and contact the appropriate healthcare professional or emergency services when needed.

Replacement coverage is subject to staff availability and cannot be guaranteed. Your contingency plan should include safe alternatives if replacement staff cannot be secured. After-hours support does not replace emergency services; call 911 in an emergency.
