Shared Assisted Living
As Canada's population ages, many older adults find themselves in a difficult position. They may no longer be able to live safely on their own, yet they do not require the level of care provided in a long-term care home. Families are often left searching for an option that offers safety, personalized support, and dignity without the institutional environment of traditional facilities.
To address this growing need, York Home Care Inc. is developing a Shared Assisted Living Program—an innovative, nurse-led model of care designed for older adults who require daily assistance while maintaining as much independence as possible.
Our vision is to create a welcoming, home-like environment where residents receive individualized care, meaningful companionship, and professional healthcare support tailored to their unique needs.

Who Is Shared Assisted Living Designed For?
Our Shared Assisted Living Program is ideal for individuals who:
-
Can no longer live safely alone

-

-
Prefer a smaller, home-like setting instead of institutional care
A Home, Not an Institution

Our goal is to help residents maintain their independence while benefiting from the reassurance of professional care available every day.
Residents can enjoy:
-
Private or semi-private accommodations
-
Comfortable shared living spaces

Daily social interaction and companionship

Every aspect of the program is designed to promote dignity, independence, and quality of life.

Supporting Families with Peace of Mind
Caring for an aging loved one can be both rewarding and challenging. Many families balance caregiving with work, raising children, and other responsibilities, often leading to physical and emotional stress.
Our Shared Assisted Living Program is designed to ease that burden by providing compassionate, professional care in a safe environment where families can feel confident that their loved one is receiving the attention and support they deserve.
We believe families should spend their time creating meaningful moments together—not worrying about whether essential care needs are being met.
An Ideal Step Between Home and Long-Term Care
Shared Assisted Living fills an important gap in the continuum of care by offering an intermediate level of support for individuals who need more assistance than independent living can provide but do not yet require admission to a long-term care home.
This model may be an appropriate option for:
-
Recovery after hospitalization
-
Progressive frailty
-
Early-stage dementia
-
Chronic health conditions requiring regular monitoring
-
Caregiver burnout or respite
-
Transitional care while awaiting other services or future living arrangements
By providing the right level of support at the right time, we aim to help residents maintain their independence, reduce unnecessary hospital visits, and enhance their overall quality of life.
